आप सभी विद्यार्थियो को कुछ दस्तावेजो को  स्कैन करके अपलोड  करना होगा जो कि, इस प्रकार से हैं –

• • Passport size Photo in jpg format (upload size is 50 KB only, of 100DPI)(पासपोर्ट साइज फोटो- अभ्यर्थी अपना वर्तमान रंगीन पासपोर्ट साइज फोटो (JPG Format, 50KB, 100 DPI) में अपलोड करें।)
  • Scanned Signature in jpg format (upload size is 50 KB only, of 100DPI) (हस्ताक्षर- अभ्यर्थी अपने स्पष्ट हस्ताक्षर स्कैन कर (JPG Format, 50KB, 100 DPI) में अपलोड करें।)
  • Left and Right Index Finger Impression in jpg format (upload size is 50 KB only, of 100DPI). In case any deformity on loss of index finder, the applicant may provide the thumb on any other finger. Impression of respective hand and continue to use the same throughout the process.(बायें एवं दायें तर्जनी छाप-अपने बायें एवं दायें हाथ की तर्जनी (Left & Right-hand Index finger) की छाप को स्कैन कर (JPG Format, 50KB, 100 DPI) में अपलोड करें। यदि किसी अभ्यर्थी की तर्जनी नहीं है तो ऐसे अभ्यर्थी उक्त हाथ की किसी अन्य ऊंगली अथवा अंगूठे का उपयोग करें एवं सम्पूर्ण प्रक्रिया की समाप्ति तक चयनित ऊंगली अथवा अंगूठे का उपयोग करना सुनिश्चित करें।)
  • Proof of Date of Birth (जन्मतिथि सत्यापन)
    • High School mark sheet or Certificate is required as proof of Date of Birth(Either mark sheet or certificate, mentioning the date of birth is to be uploaded) (जन्मतिथि के सत्यापन हेतु हाईस्कूल अंकतालिका अथवा प्रमाण पत्र जिसमें भी जन्मतिथि उल्लिखित हो को अपलोड करें।)
  • Photo ID proof (फोटो सत्यापन)
    • Any one of these documents i.e. Voter ID, Driving License, Passport, ADHAAR or any other (Please Mention) to be uploaded as photo ID Proof. (मतदाता परिचय पत्र/ड्राइविंग लाइसेन्स/पासपोर्ट/आधार कार्ड अथवा अन्य कोई वैध दस्तावेज फोटो सत्यापन हेतु अपलोड करें।).
    • The certificate number mentioned in the document (Photo ID Proof) is to be entered at the designated place in the online application form.(पहचान पत्र (फोटो पहचान पत्र) में अंकित पहचान पत्र संख्या को ऑनलाइन आवेदन पत्र में निर्धारित स्थान पर दर्ज करें।)
  • Caste Certificate (जाति प्रमाण पत्र)
    • The number on the caste certificate issued by the competent authority is to be entered and scanned copy of the certificate is required to be uploaded to avail the benefit of reservation by the OBC, SC and ST applicants. In case of any mismatch between caste certificate number entered and that in the uploaded image of the same, the application will be rejected.(जाति प्रमाण पत्र पर अंकित प्रमाण पत्र संख्या आवेदन फार्म मे यथास्थान भरा जाना सुनिश्चित करें।प्रमाण पत्र क्रमांक अपलोड की हुई स्कैन कापी में अंकित क्रमांक से मिलान सुनिश्चित करें, मिलान न होने पर आवेदन निरस्त कर दिया जायेगा।)
    • OBC applicants of Non-Creamy Layer should have a caste certificate issued on or after 1st April 2020. Validity of this certificate will be verified from the Government website.(अन्य पिछड़ा वर्ग (नॉन क्रीमीलेयर) के अभ्यर्थी का 01 अप्रैल, 2020 अथवा उसके पश्चात् जारी प्रमाण पत्र ही मान्य होगा।)
    • OBC applicants of Creamy Layer should have a caste certificate issued on or after 1st April 2020. Validity of this certificate will be verified from the concerned Government website. (अन्य पिछड़ा वर्ग (क्रीमीलेयर) के अभ्यर्थी का 01 अप्रैल, 2020 अथवा उसके पश्चात् जारी प्रमाण पत्र ही मान्य होगा।)
    • The Scheduled Caste, Scheduled Tribe, Other Backward Class and Economically Weaker Section (EWS*) applicants originally belonging to Uttar Pradesh will be given the benefit of reservation.(केवल उ0प्र0 के मूल निवासी अभ्यर्थियों को ही अनसूचित जाति, अनुसुचित जन जाति, अन्य पिछड़ा वर्ग एवं आर्थिक रूप से कमजोर वर्ग का आरक्षण देय होगा।)
    • Scheduled Caste, Scheduled Tribe, Other backward class and Economically Weaker Section applicants of other states will be treated as General candidates (UR). (अन्य राज्यों के सभी अभ्यर्थी सामान्य श्रेणी में आवेदन करेगें)

  • Applicants are required to enter the Income Certificate Number at the time of filling up the Application form. (आय प्रमाण पत्र का क्रमांक यथास्थान भरा जाना सुनिश्चित करें तथा प्रमाण पत्र की स्कैन कॉपी अपलोड करें। प्रमाण पत्र क्रमांक अपलोड किये हुई स्कैन कापी में अंकित क्रमांक से मिलान सुनिश्चित करें, मिलान न होने पर प्रमाण पत्र निरस्त कर दिया जायेगा।)
  • OBC applicants of non-creamy layer should have an Income Certificate issued on or after 1st July 2022.( अन्य पिछड़ा वर्ग (नॉन क्रीमीलेयर/क्रीमीलेयर) के अभ्यर्थी का 01 अप्रैल, 2022 अथवा उसके पश्चात् जारी प्रमाण पत्र ही मान्य होगा।)
  • Certificates and the related details if NOT found on the Government website will not be accepted. (In case of any mismatch between the Income certificate Number entered and that in the uploaded image of the same, the application will be rejected.) (प्रमाण पत्र अधिकारिक वेबसाइट पर सत्यापित किये जायेंगे। सत्यापित न होने पर आवेदन निरस्त कर दिया जायेगा।)( यदि पूरित आवेदन फॉर्म में अंकित आय प्रमाण पत्र संख्या एवं अपलोड किये गए प्रमाण पत्र में अंकित संख्या में भिन्नता पाए जाने पर आवेदन फॉर्म को निरस्त कर दिया जायेगा)
• Certificates for Weightage Claimed (भारांक)
  • Applicant has to upload the certificate related to the weightage claimed and such certificate should be duly signed by the competent issuing authority. In case an applicant fails to upload the required certificate in proper format, the weightage claimed by the applicant will not be accepted.(भारांक हेतु अपलोड किये गए सभी प्रमाण पत्र सक्षम अधिकारी द्वारा हस्ताक्षरित होने पर ही मान्य होगे। भारांक हेतु प्रमाण पत्र का वैध प्रारूप में अपलोड न होने की स्थिति में अभ्यर्थी द्वारा अनुरोध किये गये भारांक नहीं दिये जायेगें।)
• Documents related to Sub Category (उप श्रेणी)
  • Applicants have to upload the certificate related to his/her Sub-Category and it should be duly signed by the competent issuing authority. In case an applicant fails to upload the certificate, or the certificate is not in proper format, the benefit of Sub Category claimed by applicant will not be given.(उप श्रेणी में आरक्षण हेतु अपलोड किये गए सभी प्रमाण पत्र सक्षम अधिकारी द्वारा हस्ताक्षरित होने पर ही मान्य होगे। प्रमाण पत्र का वैध प्रारूप में अपलोड न होने की स्थिति में अभ्यर्थी द्वारा अनुरोध किये गये आरक्षण पर विचार नहीं किया जायेगा।)
