- उसी Official web page पेज पर या इस Article के इसी पेज पर नीचे उपलब्ध “Application format” link को क्लिक करें और उसका प्रिंट आउट निकाल लें। OR,
- Application form to be typed on A-4 size paper, should be filled in English Capital (Block) Letters using Black or Blue ink only.
